Navigating an NDIS plan can feel confusing, especially when you are trying to understand your funding, compare providers and decide which supports may work best for you.

Rise Again Collective provides person-centred Level 2 Support Coordination and Level 3 Specialist Support Coordination, depending on the complexity of support required. We help participants better understand their plans, explore available options and build confidence in managing their supports.

What Is Support Coordination?

Level 2 – Support Coordination
Support Coordination helps you understand and use your NDIS plan more effectively. It can include connecting with suitable service providers, coordinating different supports and helping you make informed decisions based on your needs, preferences and goals.

Level 3 – Specialist Support Coordination
Specialist Support Coordination is for participants with more complex support needs and circumstances. It provides a higher level of coordination to help manage complex support arrangements, address barriers and strengthen the participant’s support environment.

Our role is to provide clear, practical guidance while respecting your right to remain involved in every decision about your support.

Who Is It For?

Our Support Coordination and Specialist Support Coordination services may be suitable for participants who:

  • Are starting a new NDIS plan
  • Feel overwhelmed or unsure about how to use their funding
  • Need help finding and connecting with providers
  • Are changing existing supports
  • Have several services that need to work together
  • Want to build confidence in managing their supports more independently
  • Have complex support arrangements or circumstances requiring a higher level of coordination

We also work alongside families, carers, nominees and other members of a participant’s support network, with the participant’s consent.

How Support Is Delivered

Support may include scheduled consultations, phone or video conversations, provider research, service coordination and communication with relevant people involved in your supports, including coordination of more complex support arrangements where required.

The way we work together will be discussed with you before services begin and documented in your service agreement.

Fees and NDIS Funding

Support Coordination and Specialist Support Coordination may be funded through an NDIS plan when included in the participant’s approved supports. Fees, service arrangements and available funding will be discussed before services begin.

Eligibility and funding decisions are made by the NDIA.
